Submitting a Monthly Produce Perks Invoice

Navigate to the PPM Electronic Invoice Form - <https://www.tfaforms.com/5217833>

Enter your market name, month and year for which you are reporting.

The invoice will display data points related to the programs offered in your area. Complete all required fields with monthly aggregate totals, even if they are 0.

In the File Upload section, select a file or files that verify your monthly distribution and redemption. For most markets, you will upload your market's version of the[PPM Distribution Redemption template](https://scribehow.com/viewer/Produce_Perks_Midwest_2026_Data_Collection_And_Reporting__q4PTwNYRQwubbM-0sB2Vbw).

Select your file from the popup window.

Click "Open"

Complete the "Submitted by \*" fields and click the 'Submit' button.
